Scheduled Monitoring

                                 The frequency of visits is authorized by Client after the initial assessment evaluation.

                                 Weekly report transmitted by e-mail is included in each package as standard

         Standard Packages

• Daily M-F monitoring on an agreed schedule set by Client
  and Resident.
• Twice weekly monitoring on an agreed schedule set by Client and Resident.
• Weekly monitoring on an agreed schedule set by Client and Resident. 
• Bi-weekly monitoring on an agreed schedule set by Client and Resident. 

              Report Options

• Weekly report transmitted by e-mail is included in each package as standard.

• Weekly report transmitted by fax

• Weekly report transmitted by phone requires pre-set time. All calls during normal business hours, EST

                     Packages range in fee structure from less than $50.00 per month to

                     a maximum of under $800.00 monthly, depending on the services selected.

             Secure Adult Family Environment (S.A.F.E.) will be responsible for the completion  of all activities  based the evaluation form and as authorized by the Client at contract initiation.  The recommendations provided by S.A.F.E. following the 

         evaluation are to be used only as a guideline and only services authorized in writing by

         the Client are the obligation of S.A.F.E.
